Vincent Belcastro, M.D., FACS
Dr. Belcastro was born in New York. He attended Villanova University in Pennsylvania where he graduated with a Bachelor of Science in Biology in 1967. He received his medical degree in 1971 from the Stritch School of Medicine at Loyola University in Illinois. Dr. Belcastro performed his surgical residence at the University of Pennsylvania from 1972-1978. He established a practice in Ft. Myers in 1978 and joined Associates in General and Vascular Surgery in 1998. He is a fellow of the American College of Surgeons, Lee County Medical Society, and a member of the American College of Nutrition.
   
Lea Blackwell, M.D.
Dr. Blackwell is originally from Panama City, FL. She attended the University of Florida where she graduated with a Bachelor of Science in Zoology in 1996. Upon graduation she attended Louisiana State University where she received her medical degree in 2002 and completed a surgery residency program in 2007. Dr. Blackwell then traveled to Washington D.C. to complete a surgical breast oncology fellowship at Washington Hospital Center. She is a member of the American College of Surgeons, the Society of Surgical Oncology, and the American Society of Breast Disease. Dr. Blackwell has been with Associates in General and Vascular Surgery since 2008, where she specializes in breast cancer treatment and monitoring high risk patients for evidence of breast disease.
   
Charles Boggs, M.D.
Dr. Boggs is a Major in the United States Army Medical Corps who joined AGVS in 2008. In 1997 he received a Bachelor of Arts in Pre-Medical Sciences at Catawba College in North Carolina, followed by his Doctorate of Medicine in 2002 from St. Matthew's University School of Medicine in Belize. Dr. Boggs completed a general surgery residency program at St. Elizabeth Health Center in Youngstown, Ohio in 2007, after which he journeyed to Tampa, Florida to complete a burn surgery fellowship. He is active in Toys for Tots, the United States Marine Corps annual Christmas toy drive. Dr. Boggs also enjoys open water diving, running, and his continued studies of the Spanish language.
   
Thomas Carrasquillo, M.D., FACS
Dr. Carrasquillo has maintained a private practice in the Ft. Myers area since 1980 and joined AGVS in 2009. He received a Bachelor of Science in Biology from St. Edward's University in Texas in 1968, followed by his medical degree in 1972 from Cardinal Stritch School of Medicine at Loyola University in Illinois. Dr. Carrasquillo completed his residency program in 1978 at the University of Illinois Medical Center in Chicago. He is a member of the Lee County Medical Association, the Hispanic-American Medical Association, the Florida Medical Association, the Warren H. Cole Surgical Society, and is a Fellow of the American College of Surgeons. Dr. Carrasquillo serves on the Cape Coral Hospital Medical Executive Committee and has been Chairman of the Department of Surgery and Chief of Staff there. During his early medical career he was a Major in the United States Army. Dr. Carrasquillo and his wife are avid missionaries, traveling to the Dominican Republic yearly to treat patients in need.
   
Thad Kammerlocher, M.D., FACS
Dr. Kammerlocher is originally from Oklahoma. He graduated from the University of Oklahoma in 1988 with a Bachelor of Science in Zoology and Psychology. He continued his education at the University of Oklahoma and earned his medical degree in 1992. Following graduation he interned in surgery at the Baptist Medical Center in Birmingham, Alabama. Dr. Kammerlocher then completed his residency in surgery at the Orlando Regional Medical Center in 1998 when he was also recognized as Resident of the Year. He pursed a fellowship in vascular surgery at Vanderbilt University from 1998-1999. Since that time Dr. Kammerlocher has been a part of Associates in General and Vascular Surgery. His special interests include minimally invasive surgeries and vein surgery and treatments. He is a member of several societies including: Lee County Medical Society, a Fellow of the American College of Surgeons, and the American Medical Association. In his free time Dr. Kammerlocher enjoys golfing, baseball, and spending time with his family.
   
Thomas Kowalsky, M.D., FACS
Dr. Kowalsky was born in Pennsylvania. He attended Washington and Jefferson College where he graduated in 1975 with a Bachelor of Arts in Biology and Chemistry. He briefly attended the University of Pittsburgh graduate school for chemical engineering before deciding to attend medical school at Temple University. It was there in 1980 that he graduated with his Doctorate of Medicine. Dr. Kowalsky then completed an internship at Mercy Hospital in Pittsburgh, PA, followed by a general surgery residency from 1981-1985. From 1985-1986 Dr. Kowalsky carried out a vascular fellowship at the Hope Heart Institute in Seattle, Washington. Since 1986 he has been a part of Associates in General and Vascular Surgery in Ft. Myers. He is a member of several societies including: Lee County Medical Society, Florida Vascular Society, Florida Medical Society, a Fellow of the American College of Surgeons, and the Society of Vascular Surgery. He has also served on several of these boards and as President of the Medical Staff at Lee Memorial Hospital and Chief of Staff. Dr. Kowalsky enjoys spending time with his wife and children.
   
Michelle Mon, M.D., FACS
Dr. Michelle Mon is originally from Iowa. She completed her undergraduate studies with honors and distinction at the University of Iowa in 1995, earning a Bachelor of Science in Biology. Dr. Mon then continued her education at the University's College of Medicine, attaining her M.D. in 1999. During her time in school she interned in trauma surgery, cardiac intensive care, and burn surgery. She completed her general surgery residence at the University of Iowa Hospitals and Clinics in 2004. Dr. Mon has been a member of Associates in General and Vascular Surgery since 2006.
   
Physician Assistant  
Dan Beitelschies, PA-C
Dan joined Associates in General and Vascular Surgery in 2007 as our Physician's Assistant and previously worked with a local cardiac surgical group beginning in 1989. He attained a Bachelor of Science in Physician's Assistant from Mercy College of Detroit in Detroit, Michigan in 1979. He worked as a PA in Michigan for several years before moving to Florida. From 1998-2004 Dan was a professional consultant/trainer for Johnson and Johnson/Ethicon Inc. He has been a senior clinical specialist for Terumo Cardiovascular Systems, performing presentations and conducting training sessions. Dan is a member of the American Academy of Physician's Assistants and the Florida Academy of Physician's Assistants. He enjoys spending time with his family and is an avid member of the United Schutzhund Clubs of America.
